Sign Up Without Activation Code

It’s interesting what you can do with a registered and activated My Duke Health account. You will need an unused Activation Code to be able to access the health mychart portal.

In the absence of an Activation code, you will need to ask for a new one. How do you request for a new code? it just by signing up online with your personal information and email address to receive the new code.

How To Request A New Activation Code and Sign Up

Here’s an overview of how you can get a new activation code and create an account on My Duke Health MyChart portal.

Step 1: To sign up online and receive a new activation code, you must be a fresh user of My Duke Health or the former Duke MyChart; go to (https://dukemychart.org/Home/Signup). Copy and open the URL on your browser.

Step 2: As soon as the MyChart sign-up page to request a new activation code loads in your browser, you will be instructed to complete a registration form. You must fill this form out completely.

Step 3: The registration form will require the following information from you;

  • Your Legal Full Name (First, Middle and Last Name).
  • Your Residential Address.
  • And other personal information like your Date of Birth, SSN, Phone Number, Gender and Email address.

Step 5: Verify your details one more after completing the sign-up form. Move on to the next step for form submission and contact verification if everything checks out as it should.

Step 6: Proceed to click on Next button, your activation code will be emailed to you so check your inbox or spam box for it. This then takes you to verify your identity and choose a Username and Password. Now you can continue to My Duke Health login using your newly created account since you now have it as an active account.

NOTE: This registration form is strictly for new customers who do not yet have any record whatsoever on the MyChart app or the website portal. If you already have an active Activation Code, please use it on the Sign Up page for existing Activation Code and verify your account.

Your activation code is not the same as your password or MyChart ID. When logging onto MyChart for the first time, you will only need to use this code once. (Keep in mind that there is a time limit on the code, so use it before it expires). You will then be prompted to create your own special MyChart ID and password when you log into MyChart for the first time.

Duke Health MyChart Login

If you are already an account owner or user of the previous Duke MyChart, you will now have to further access your My Duke Health portal using the same login credentials. Please if you don’t have an account yet, go to “Sign Up” to register an account now.

You don’t have to worry about changing anything, use the same MyChart Username and Password. Remember that only your Password is case sensitive, you might want to be careful when entering it.

  • Go to the My Duke Health Login page.
  • Enter your correct MyChart Username and Password in the given field.
  • Finally, click on Sign In to gain access and begin navigating your patient portal.
