Go to My Duke Health Login (https://my.dukemychart.org/) and click on Forgot Password? link, this opens a fresh page to fill a password recovery form. The compulsory information you will be asked for are;
- Your MyChart Username.
- Date of Birth.
- Your address Zip Code.
If you still don’t remember your Username which is highly needed here, Retrieve your MyChart Username now and have it sent to your email. Make sure all information are submitted correctly as provided during sign up and activation of your health portal.
Click on ”Next” button to continue. If the system detect your Username and Email address on the server, it will then send you further verification link to set up a new password.
How To Login My Duke Health (Duke MyChart)
